Veterinary history for any pets in your care for the last 5 years.
Current pets should receive annual physicals. We require that pets currently living in the home are vaccinated against rabies as required by law and distemper as recommended by your veterinarian. Dogs should be on heartworm preventative and cats must be FeLV/FIV free unless adopting a cat with this condition.
If you do not have this information from your veterinarian this will cause delay in processing.
If you rent, an up-to-date copy of your lease and/or a current name and number of your property owner.
Failure to provide this will result is delay of application
Any application pending for 30 or more days due to lack of information will be purged. Upon updating your information we welcome you to reapply!
